A detailed statement of your bank account covering a six-month period immediately prior to your visa application, and showing sufficient funds to cover your costs 7. House Registration Book of the applicant in Thai + English translation, land title deed in Thai+ English translation (if any) 8. Details of any family members living in your country. If you are married or have children under age eighteen, and your spouse or children are remaining at home, you should submit your marriage certificate, house registration book and birth certificates for the children in Thai + English translation. 9. Where a child under the age of 18 is travelling alone, the consent of both parents/guardians is required. -If the child is travelling with one parent, the consent of the other parent is required. This signed parental consent must be accompanied by a copy of the consenting parent's passport or national identity card showing the bearer's signature. - Where only one parent has total custody, a Court Order bestowing sole custody of this child must be shown. The consent of the other parent is also required. In case other parent cannot be contacted, letter of explanation should be provided. 10. Provisional flight Booking, Please do not purchase travel tickets prior to a decision being made on the visa application. Doing so, may lead to unnecessary additional costs in circumstances where a visa is not granted. 11. Hotel Booking 12. Evidence of insurance cover will help us decide your application. Letter from your employer or Association confirming your attendance, and outlining the relevance of the conference to your employment or involvement in your Association. 7. Evidence from both your employer and conference host showing who will be responsible for all cost including travel, accommodation, expense etc. 8. Sponsor’s bank statement (If your employer will be responsible for all cost of this trip) 9. Applicant’s bank statement covering 6 months period immediately prior to your application is required (If you are paying these cost yourself) 10. Evidence of your obligations to return to your country of permanent residence (letter from your employer/college showing the date of expected return, House registration book (Thai + English translation) etc. 11.
